- Five (5) original copies duly accomplished ERF
- One (1) original copy CSC For, 212 (PDS)
- One (1) certified true copy latest Appointment
- One (1) original copy updated Service Record
- One (1) certified true copy TOR with CAV indicated as CAR/Graduate in Masteral Degree from CHED
- Teacher II - BSEE/BSE or Bachelors Degree w/ 18 Professional Educ. Units + 20 MA units
- Teacher III - BSEE/BSE + MA Equivalent
- One (1) certified true copy of Performance Ratings (IPCRF) for the last 3 school years
- One (1) certified true copy renewed PRC ID
- One (1) clear copy authenticated by AO or Head of Agency of the following, if applicable
- Trainings, seminars, workshops (not used during the last promotion)
- Teaching experience in private schools prior to assignment in Deped (certificate of employment)
- Administrative/Supervisory designations
